Step 1:
Call or Email Us

First, call or email and notify us of the death. We will then be able to inform you what documentation is necessary, the death benefit amount, and who the beneficiary is.

Step 2:
Submit Documentation

In order to process the claim, we need the following information:

Death Claimant’s Statement, signed by beneficiary. Download Claims Form.
Photocopy of Death Certificate.
You may need further documentation for your claim.
Paperwork can be returned to us via fax, mail, or email.

Receive Payment
